Adjunct/Overload Request Form

For full consideration, deadline for submission of this form to the Office of Academic Affairs: Monday, September 21st. Please submit a separate form for each adjunct/overload taught course (multiple sections per form allowed).

Please include information on: staffing issues created by course releases, department FYP staffing, sabbatical and FYP leaves, etc.; students served and/or purposes of the course in the university and department curriculum; any other special circumstances or information leading to this request.
