Information for Varsity Athletes

Pre-Participation Forms:  If you are a varsity student-athlete please go to the Sports Medicine webpage on the St. Lawrence Athletics website to complete all required medical forms to participate in intercollegiate athletics at St. Lawrence University.  A link to the website is here: http://www.saintsathletics.com/sports/2013/7/24/medicine.aspx?tab=sportsmedicine

Health Insurance: The NCAA requires all participants in NCAA sports to provide health insurance coverage information as requested on the university’s health insurance waiver form, as well as evidence of insurance showing a minimum of $90,000 coverage for athletic related injuries.  The University’s athletics department highly recommends that every student -athlete enrolls in the Student Health Insurance Plan (SHIP) because of the extensive coverage for athletic related injuries that may not be covered by family insurance.

SHIP required of all St. Lawrence students (unless waived via proof of equivalent insurance) provides benefits for the NCAA mandated minimum of $90,000 of eligible expenses for injuries resulting from intercollegiate sports sponsored by the University and will cover 100 percent of NCAA sport related injuries up to $90,000 with $0 deductible.  Note:  St. Lawrence will not be held responsible or liable for any incidental out of pocket NCAA sport injury related expenses, deductibles, co-payments, etc.

The university also carries a Catastrophic injury insurance policy provided by the NCAA which provides up to $20 million in lifetime benefits to varsity student athletes who are catastrophically injured during qualifying intercollegiate athletics play or practice, or during travel related to these activities.  It provides benefits for medically necessary care and treatment in excel of all other available benefit plans.  Details of this program are available at the Augsbury Physical Education Center Office.
