What to expect when completing Verification Form A

Verification Form A must be completed by all Biomedical Sciences majors regardless of the type of Experiential Learning Component (ELC) completed.  This form asks you to provide some basic information about what you did, but also requires you to provide a Reflection Statement on the experience.  Incomplete submissions will be discarded, so please work carefully through this process.
To make this process most streamlined and effective, we ask that you gather the information below BEFORE accessing the Verification Form.  The items listed below are what you will be asked to answer when you click the link:

ALL students will be asked the following:

Student Information: your name, e-mail address, BMS major advisor’s name
Reflection statement: Prepare this ahead of time so that you can cut/paste it into the form.  Visit the ELC webpage for lots of info on the expectations for this statement.  The brief prompt provided within the Verification Form is shown here:
Submit your experiential learning component (ELC) reflection statement (500-1000 words) below. You may want to consider addressing some of the following prompts:
•    How did your experience connect to the learning goals of the Biomedical Sciences major?
•    Did you apply knowledge from your coursework to specific situations?
•    Review your pre-reflection notes. Reflect on your expectations coming into the experience and what you took out of the experience.
•    How can you apply what you learned in your ELC to your future career?
•    Identify ways in which your ELC can enhance your experience in the classroom as you finish your requirements for your BMS major.
•    What strengths, weaknesses, and skills did you discover through this experience?


Students who completed a credit-bearing experience will also be asked the following:

Health Coaches: community placement for Health Coaches I
SYE: project mentor; department sponsoring the SYE; semester the SYE project was completed; unit value for the SYE


Students who completed a non-credit bearing experience (options listed below) will also be asked the following information:

•    Emergency Medical Technician (EMT) training*
•    Certified Nursing Assistant (CNA) training*
•    Certified Phlebotomy Technician (CPT) training*

•    name and e-mail address of mentor/supervisor
•    approximate dates of experience completion (start/end dates)
•    location of experience
•    brief description of tasks completed
•    number of hours per week, number of weeks, number of total hours spent for the experience

*For students who completed EMT, CNA, or CPT training : You will be also asked to provide the date on which you submitted your certificate/licensure to Biology Department secretary Shanda Beamer (sbeamer@stlawu.edu) for scanning.
